What is Medicare Part A?

Medicare Part A is hospital insurance that covers inpatient care, providing support for services like hospital stays, skilled nursing facility care, limited home health services, and hospice care.  It’s designed to make essential healthcare more accessible and affordable for Medicare-eligible individuals.  Understanding what Part A covers—and what it doesn’t—can help you effectively plan for potential healthcare expenses.

Coverage Highlights of Medicare Part A

Here’s a quick breakdown of what Medicare Part A covers:

  • Inpatient Hospital Services

When admitted as an inpatient on a doctor’s recommendation, Medicare Part A covers:

    • A semi-private hospital room
    • Meals
    • General nursing care
    • Medications and necessary hospital supplies
  • Skilled Nursing Facility (SNF) Care

If you require daily skilled nursing or rehabilitation services following a hospital stay, Medicare Part A helps cover:

    • Semi-private rooms
    • Meals
    • Skilled nursing and therapy (physical, occupational, etc.)
    • Medical supplies
  • Home Health Care Services

If you’re homebound and need skilled care, Part A might cover:

    • Part-time nursing care
    • Physical, occupational, and speech therapy
    • Health aide visits (part-time)

Care must be part of a doctor-approved treatment plan.

  • Hospice Care

Designed for individuals with a life expectancy of 6 months or less, hospice care under Part A includes:

    • Pain management and symptom control
    • Medical and nursing services
    • Social and emotional support

Available care settings ─ home, hospice facilities, or Medicare-approved hospitals, depending on your needs.

  • Inpatient Rehabilitation Services

If you require intensive rehabilitation after an illness or injury, Part A covers inpatient rehab services at specialized facilities, such as rehab hospitals or long-term care hospitals.  Therapy must generally consist of at least 3 hours per day, 5 days a week.

What Medicare Part A Does Not Cover

While Part A provides significant inpatient coverage, there are gaps you should be aware of:

  • Long-Term Custodial Care: Covers skilled nursing but not personal care needs like bathing or eating. Long-term care insurance might help fill this gap.
  • Outpatient Services: Doctor visits, X-rays, and lab tests typically fall under Part B (medical insurance), not Part A.
  • Most Prescription Drugs: Prescription coverage generally requires Medicare Part D or a Medicare Advantage plan that includes drug coverage.

Eligibility & Enrollment

Many individuals are automatically enrolled in Part A when they turn 65, especially if they’re already receiving Social Security benefits.  If you’re not automatically enrolled, you’ll need to sign up during your Initial Enrollment Period (IEP), which starts 3 months before and ends 3 months after your 65th birthday.

Financial Considerations Under Part A

If you or your spouse paid Medicare taxes for at least 10 years, there’s no monthly premium.

  • Lifetime Reserve Days (beyond 90 days in hospital): Medicare provides 60 reserve days to use during your lifetime.  After they’re used, you’re responsible for the full cost of additional hospital stays unless you have Medigap coverage (e.g., Plan G provides an extra 365 hospital days).
  • Deductibles & Coinsurance: Part A deductibles and cost-sharing may require supplemental plans or savings to manage expenses.

Final Thoughts

Medicare Part A is a tremendous tool for managing inpatient care but comes with limitations that require careful consideration.  To better protect yourself against gaps in coverage, you’ll want to explore plans like Medicare Part B, Part D, or Medigap supplemental insurance.
